A "packer" is pulled through the sewer line and stopped at each joint. If the joint holds pressure it passes the pressure test. If the joint fails to hold pressure a chemical grout is injected into the area to seal the joint (or crack). The process is remotely monitored and controlled from the surface. In very large diameter pipe repairs are performed by a man entry crew.
